Description:Klickitat Valley Health is now seeking a caring, compassionate and self-motivated CNA/ Activities Coordinator to join the Acute Care Department. This full-time position offers one 12 hour CNA day shift (0700-1930) plus two 10 hour day shifts (0800-1830) as Activities Coordinator, 32 hours per week (weekdays will vary).

This Full-time position is eligible for benefits including Medical, Dental, Vision, Rx, Retirement, Life Insurance, Long/Short Term Disability and generous Paid Time Off (PTO).

The Certified Nursing Assistant (CNA) provides basic nursing care activities under the direction of the Charge Nurse (RN), Registered Nurse (RN), Licensed Practical Nurse (LPN), provider, etc. in the MS Unit within his/her scope of practice (RCW 18.360.050). The CNA assists the provider, RN’s, LPN or other nursing staff with: daily care of the patient, collecting patient information, communicating key information to the RN, LPN or provider, and anticipates/acts on needs of the patient proactively.

The Activities Coordinator develops, implements and maintains a comprehensive activity program for the “Swing Bed” patients of the hospital on the MS Unit. The Activities Coordinator plans a variety of activities for the patients in order to give them opportunities to use and maintain their physical and mental abilities, provide outlets for social interaction, etc. Keeps nursing staff aware of patient’s condition and records information in the patient’s chart/record of activates participated in and any general observation regarding the patient’s condition. The Activities Coordinator demonstrates excellence in customer service during interactions with patients, families and works collaboratively with colleagues to provide optimal outcomes for patients in a compassionate, professional, ethical and knowledgeable fashion. The Activities Coordinator is aware of the potential exposure to internal and external hazardous materials, including but not limited to: blood and blood borne pathogens, infectious/communicable diseases, potential physical injury while caring for patients. Maintains a safe and clean environment for the care of the patient.
Duties:• Provides basic nursing care for patients including, but not limited to: vital signs, personal hygiene/skincare, ADL’s, monitors Intake and Output (I&O), makes/changes bed linen, answers call lights to determine patient needs, ambulates/assists patient as needed, lifts/turns/moves patient as needed following proper procedure, etc. to ensure quality patient care.
• Provides care appropriate for the condition and age of the patient, including: pediatric, geriatric and general population and maintains awareness of patient needs based on age specific factors, as well as being aware of the patient’s physical and psychosocial needs.
• Reports any changes in patient status or condition to the supervising RN, physician/provider, etc. to ensure patient comfort and safety, notifies the RN/physician/provider when patient complains of pain or discomfort.
• Documents all patient care activities and changes in patient condition consistent with nursing policy and procedure; gives detailed report at the end of shift/assignment to supervisory RN and/or other nursing staff.
• Supports the mission, vision and values of the organization.
• Provides individual or group activities for “Swing Bed” patients including, but not limited to: exercise programs, walking/ambulation assistance, card/table games, puzzles, bingo, crafts, reading materials and/or assistance, music (radio/CD), etc. in order to provide the patient with physical and mental stimulation and social interaction opportunity.
• Develops daily activity programs calendar and posts/distributes information in order that patients and nursing staff are aware of planned activates; organizes the activity event(s) or activity materials/supplies; monitors patient(s) during activity to ensure safety. Provide special recognition events (if appropriate) such as: birthdays, anniversaries, etc.
• Provides care appropriate for the condition and age of the patient, including: geriatric and general population and maintains awareness of patient needs based on age specific factors, as well as being aware of the patient’s physical and psychosocial needs.
• Completes documentation of all patient activities daily and reports any changes/concerns in patient status or condition to the supervising RN, physician/provider, etc. to ensure patient safety consistent with nursing policy and procedure.
• Assists patient (if needed) at meal times.
• Assists medical/surgical (MS) CNA’s with ADL’s to include: bathing, showering, dressing, toileting, linen changes, vital signs, etc.
• Documents daily activities, when the Activities Aide is not available then activities will be provided by the medical/surgical (MS) staff.
• Completes initial and annual competencies.
• Participates in quality review and performance improvement (PI) process.
• Complies with all hospital policy and procedures, while upholding the high expectations of KVH.
• Attends weekly swing bed meetings with Case Manager.
• Attends a minimum of 50% of nursing staffing meetings and 100% of mandatory in-services.
• Performs other miscellaneous duties as assigned.
Qualifications:Education / Licensure:
• High school diploma or GED required.
• Current Washington State Certified Nursing Assistant certification.
• Current BLS certification required.
• Current/valid Oregon/Washington driver’s license (depending on location of residence) w/o serious driving record violations and ability to meet KVH insurance requirements.
• Knowledge/comprehension of basic medical terminology required.
• Basic computer skills required; experience with CPSI software preferred
• Pass a DSHS background check required.
• Food handler permit required.
• Satisfactory completion of a training course for Activity Directors approved by DSHS (preferred)

Experience:
• At least one (1) year recent full time experience providing direct patient care in an outpatient or acute care medical setting required.
• Prior experience working with elderly persons, with ability to relate in a positive, supportive manner essential.

Skills/Knowledge/Abilities:
• Demonstrates knowledge of basic CNA skills (i.e., taking vital signs, recording I/O, etc.) first aide; patients’ rights; universal precautions; infection prevention; body mechanics; basic medical and nursing terminology.
• Requires good thinking/reasoning skills, ability to make good decisions, listen and accept advice from others, demonstrate maturity and good judgment and the ability to work with regular supervision.
• Perform various sensory requirements such as: vision, speech, touch, manual dexterity, fine motor skills and hearing in order to assess and treat the patient accurately.
• Promote/deliver excellent customer service to patients, families and staff.
• Demonstrate positive and effective interpersonal and communication skills with colleagues, patients, families, providers, volunteers, etc.
• Work productively, cohesively within a team and resolve conflict/differences in a positive manner.
• Work in a stressful/demanding environment and take appropriate action to ensure quality of patient care.
• Demonstrate attention to detail, follow oral and written instructions precisely, and maintains awareness of patient’s condition while participating in activities.
• Ability to read, write, speak and understand English.
• Experience with using everyday computer electronic tools (e.g., email, data retrieval, voicemail, and internet/intranet) as well as ability to effectively utilize an electronic medical record (EMR) to record/report on the patient’s activity involvement.
