Klickitat Valley Health is now seeking a caring, compassionate CNA to join the Home Health & Hospice departments. This part time position offers 20 hours per week, day shift Monday-Friday.
The Certified Nursing Assistant (CNA) provides personal nursing care under the direction of the Director of Home Health & Hospice, staff Registered Nurses (RN) and the patient’s physician/provider, etc. in the home setting within his/her scope of practice (RCW 18.360.050).The CNA provides direct patient care, collects patient information, communicates key information to the RN, or physician/provider. The CNA demonstrates excellence in customer service during interactions with patients and families and works collaboratively with colleagues to provide optimal outcomes for our patients in a compassionate, professional, ethical and knowledgeable fashion. The CNA is aware of the potential exposure to internal and external hazardous materials, including but not limited to: blood and blood borne pathogens, infectious/communicable diseases, potential physical injury while caring for patients.
The Home Health & Hospice team cares for patients that are homebound and/or at the end of life.
• Provides personal nursing care to patients including, but not limited to: vital signs, personal hygiene/skincare, ADL’s, nail care, catheter care and/or enema care, monitors Intake and Output (I&O), makes/changes bed linen and “tidy up” the patient’s room, ambulates/assists patient with exercises as needed, positions/lifts/turns/moves patient as needed following proper procedure, etc. consistent with the care plan established for the patient and to ensure quality patient care, as directed/supervised by primary care nurse (PCN) and physician/provider orders.
• May apply topical creams/ointments/solutions for nose/eye/ear applications or to intact but reddened skin areas as directed by PCN or physician/provider orders; additionally remind patient when to take medications and may read Rx label(s) for patient, as well as open medication container(s) if assistance is needed.
• Instructs patient regarding the use of durable medical equipment (DME).
• Provides care appropriate to condition and age of the patient, including geriatric and general population and maintains awareness of patient needs based on age specific factors, as well as being aware of the patient’s physical and psychosocial needs.
• Reports any changes in patient status or condition to the PCN/physician/provider, etc. to ensure patient comfort, needs and safety are achieved, and notifies the PCN/physician/provider when patient complains of pain or discomfort or has other requests.
• Properly documents all patient care activities and changes in patient’s condition consistent with departmental policy and practice; gives detailed report as needed to the supervisory RN.
• Consults with other department staff as appropriate to provide for interdisciplinary approach to patient care; and communicates clearly (both written and verbal) with PCN/physician/provider to ensure the care needs of the patient are met.
• Follows KVH policy when disposing of bio-hazardous materials, labeling and handling of materials.
• Performs all aspects of personal nursing care in a safe manner and to reduce the likelihood of medication/health care errors, and is accountable for each specific patient assigned.
• May provides clerical assistance for the department, including but not limited to: filing, shredding documents, coping records, answering phones, compiling patient charts, maintaining/organizing storage room, cleans/stocks hospice kits, etc. as needed.
• Complies with all policy and procedures, while upholding the high expectations of KVH.
• Attends various staff meetings and mandatory in-services as required.
• Performs other miscellaneous duties as assigned.
Minimum Education, Training & Experience (includes licenses or certifications):
Education / Licensure:
• High school diploma or GED required.
• Current Washington State Certified Nursing Assistant certification and HCP-CPR required.
• Completion of the Checklist for Skills Demonstration established by the National Home Caring Council Foundation for Hospice and Homecare; or completion of the Medicare National Certification Homemaker – Home Health Aides assessment test with a minimum of 75% correct passing score.
• Possess a valid/current driver’s license in the State of Washington/Oregon depending on residence location.
• Possess or has access to reliable vehicle transportation with auto insurance for doing patient care visits.
• Current BLS certification required and pass TB testing.
• Knowledge of HIV/AIDS conditions and protocols.
• At least one (1) year recent full time experience providing direct patient care required.
• Demonstrates knowledge of basic CNA skills (i.e., taking vital signs, recording I/O, basic personal care, etc.) first aid, patients’ rights, universal precautions, infection control, body mechanics, basic medical terminology.
• Requires critical thinking skills, ability to make good decisions, listen and accept advice from others, demonstrate maturity and good judgment and the ability to work with regular supervision.
• Perform various sensory requirements such as: vision, speech, touch, manual dexterity, fine motor skills and hearing in order treat the patient accurately.
• Promote/deliver excellent customer service to the patient and support/assist family members in understanding the care provided to the patient.
• Demonstrate positive and effective interpersonal and communication skills with colleagues, patients, families, physicians, volunteers and community.
• Work productively, cohesively within a team and resolve conflict/differences in a positive manner.
• Work in a stressful/demanding environment and take appropriate action to ensure quality of patient care, ability to cope with both physical and emotional demands of the job.
• Maintain a calm and composed manner in emergency/stressful situations, taking appropriate action as necessary.
• Demonstrate attention to detail, follow oral and written instructions precisely, and maintain a high degree of accuracy; risk of error could result in significant harm or loss.
• Excellent organizational and multi-tasking skills with ability to anticipate tasks and prioritize assignments.
• Ability to read, write, speak and understand English.
• Experience with using everyday computer electronic tools (e.g., email, data retrieval, voicemail, and internet/intranet) as well as ability to effectively utilize an electronic medical record (EMR).
• Adaptability is essential to manage the needs of the patient with unpredictable census and resources
