Description:Are you committed to providing compassionate care to others, but don't want to commit to working a full time schedule? Klickitat Valley Health is currently seeking a Licensed Occupational Therapist to provide part time occupational therapy services to our Home Health and Hospice patients throughout Klickitat County. This position offers 2 days a week with a total of 16 hours a week.

The Occupational Therapist (OT) may be an employee or an independent contractor who provides occupational therapy services to KVH Home Health patients as ordered by the physician.

REPORTS TO: Director of Home Health & Hospice or Therapy Supervisor as assigned
SUPERVISES: Occupational Therapy Assistants for direct patient care as assigned by supervision
Duties:Each initial visit will consist of an evaluation and development of a treatment plan. Follow up visits may be necessary to meet the objectives of the patient's treatment plan.

• Demonstrates support for the agency mission statement to promote quality, efficient, comprehensive, and effective patient service.
• Participates in and supports the agency’s performance improvement activities.
• Participates in orientation, competency testing, and in-services as required.
• Performs comprehensive initial and on-going occupational therapy assessments per standards of practice on each assigned agency patient and develops the plan of care in collaboration with patient, RN Case Manager, and physician.
• Regularly evaluates the effectiveness of each assigned patient’s plan of care, response to treatment, revision needs, discharge plans, and progress to patient goals.
• Establishes goals to enhance the rehabilitative ability of patient, which may include but not be limited to: neuromuscular facilitation/inhibition; perceptual, prosthetic, and kinetic treatment; orthotics/splints; activity of daily living training; and related to needs.
• Uses a framework of assessment, implementation, coordination, and evaluation providing outcome-based services for an assigned caseload from admission to discharge.
• Complies with all home health care and hospice regulations and standards and agency policies and procedures.
• Appropriately seeks supervisory assistance as necessary to effectively perform duties.
• Assesses and reports patient pain score to RN Case Manager.
• Maintains confidentiality of patient, personnel, and agency operations.
• Using agency documentation materials and/or computer based software to record patient treatment activity in a timely manner including patient response to interventions.
• Promotes patient, personnel, and office safety and utilizes universal precautions.
• Meets productivity requirements established for home health care occupational therapist.
• Supervises assigned Certified Occupational therapist Assistants (COTAs) by establishing and/or updating a plan of care in conjunction with the patient/representative and evaluating compliance and progress to measurable goals.
• During patient/representative interactions identifies cultural, spiritual, verbal, and non-verbal patterns indicating a need for counseling and suggests referral to RN Case Manager.
• Assesses caregiver’s ability to manage patient occupational therapy needs.
• Evaluates the safety of the home environment and recommends to patient/representative potential solutions for identified concerns.
• Uses equipment properly and seeks training if necessary.
• Provides education, advice, and consultation with patient/representative and agency personnel as needed.
• Participates in QAPI quarterly record review as assigned.
• Presents a professional appearance and promotes positive work environment.
Qualifications:Washington State Occupational Therapist License and CPR/BLS required. Must be available to provide home care visits at least 2 days per week. Reliable transportation, valid driver's license, and personal automobile insurance are required.

• Must be a graduate from an occupational therapy educational program accredited by the Accreditation Council for Occupational Therapy by the National Board for Certification in Occupational Therapy, Inc. (NBOCOT), or other specified requirements of the Home Health Care Conditions of Participation 42 CFR 418.1114(b)(5)-418.114(b)(5)(v)(A)(2)
• Possesses and maintains a current state certification/license as an OT
• Maintains CPR certification
• Possesses effective communication skills and ability to work with individuals from a variety of backgrounds and educational experiences
• Possesses good time management and work ethics
• Possesses current driver’s license and a dependable insured automobile
• Possesses ability to make independent patient care judgements and is self-direction
• Presents a professional appearance and promotes a positive work environment
• Demonstrates computer literacy
• Is able to meet the physical requirements as outlined in this job description
