The Home Health & Hospice team takes great pride in providing outstanding, quality nursing care in the comfort of patients' homes throughout Klickitat County. The RN is an important member of the healthcare team and works collaboratively with the other members of the care team to provide optimal outcomes for our patients in a compassionate, professional, ethical, and knowledgeable fashion.
|Duties:||This RN position is responsible for performing systems assessments, following and adjusting as needed comprehensive, personalized, patient centered care plans that outline specific objectives and courses of treatment and developing teaching/educational plans for patient & families under physician direction on a regular basis.|
The RN will identify and address psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families and make appropriate referrals to other team members.
They will also be responsible for the delivering and documentation of all patient care as prescribed by the medical provider, considering needs specific to the standard of care for patientís age and within the scope of practice of the Registered Nurse, including: assessing/monitoring vitals, administering therapies and treatment, all types of wound care, access/maintains IV/PICC/Port lines for medication and fluids; observing the patient and recording those observations, answering patient/family questions, etc.
The RN will use sound nursing judgment to identify and communicate changes in the patientís condition to patient's physician and co-workers.
The RN will demonstrate an excellence in customer service and care during interactions with employees, managers/supervisors, physicians, patients and patient family members, etc.
The RN is expected to work collaboratively with other Home Health & Hospice staff, delegating tasks as appropriate with CNA staff, as well as supervising care as required by Medicare. The RN may perform other duties as assigned.
|Qualifications:||The RN must have the following qualifications: be a graduate of an accredited school of nursing with an active/current RN license in Washington State. BSN preferred, but not required. |
Must possess a valid/current driverís license in the State of Washington/Oregon depending on residence location and possess or has access to reliable vehicle transportation with auto insurance for doing patient care visits. BLS-CPR required and pass TB testing.
Must have knowledge of HIV/AIDS conditions and protocols.
Must be able to demonstrate attention to detail and high degree of accuracy, risk of error could result in significant harm or loss.
Must have experience with using everyday office tools (e.g., computers, email, data retrieval, voicemail, and internet/intranet), proficiency in Microsoft Word / Excel as well as ability to effectively utilize an electronic medical record (EMR), as well as use/handle all medical equipment utilized in the care of the patient.
Adaptability is essential to manage the needs of the patient load with unpredictable census and limited staff.
Must be able to take on-call rotation.
ē At least two (2) years general nursing experience required, and at least six (6) months nursing experience in Home Health and/or Hospice care preferred.
